Heh, I recently has the (dis)pleasure of getting my 2nd class air medical to begin flight training at a university (though I just changed my focus to aerospace engineering for other reasons so I won't be flying ). The standards for an airline pilot's health are reletively strict and they are age cap is 60, add in the fact that they must recieve a new medical every 6 months and the chances of a medical problem mid-flight is pretty slim. And if anything does happen, the pilot can recieve care back in the cabin while the co-pilot flies (or watches the autopilot ).
well you must admit a scenario requiring an access from the outside is probable although it's a marginal case, but still exists (I envy you about that training)

It is well known that most of "civilized" pilots know nothing about behaviour in critical situations. They are so addicted and get used to those "fly by wire" gadgets that forget what to do in case of decompression. There ARE oxigen masks, there IS an emergency descent procedure... Or they were two grandmas that lost conciousness immediatelly after decompression.
friday night a plane had made a dive and emergency safely landing in france in case of decompression. all ended ok.
About the Athens crash, the F-16 pilot who flew near the cockpit said someone was trying to regain control of the plane. Very weird crash if you ask me, I don't think you can put it down to a couple reasons, probably all systems got screwed at the same time.
From what I heard over here (I heard very little cos I was omw to Spain at the time) the whole of the cabin decompressed. A passenger sent a text msg to a friend saying they were freezing to death... Im sure I also heard something about a passenger saying the pilot had entered the passenger cabin and was blue in the face.
